SUBMISSION
& PAYMENT INFORMATION |
STEP
1: Once you hit Submit below, your application will be
sent to the MSYSA State Office. Your form will not process
if you do not enter a valid email address or if you fail
to check the appropriate verification boxes. All applications
must be received on or before 4:30pm on July 9, 2009.
STEP 2: When
you submit your application, you will be taken to a web
page that contains your "Tournament ID #" and other important
information. You should make two copies, one to keep on
file and one to mail with your entry fee.
STEP 3: Payment
of $425 must accompany this application. You may either
pay the entry fee by check, made payable to MSYSA, or
by credit card. Further payment instructions will follow
once you correctly submit this application. If you fail
to return both application and payment to the MSYSA State
Office on or prior to the established deadline, your
application will be rejected. Registrations
are not complete until payment has been received. A
complete list of accepted teams will be posted on our
web site at www.michiganyouthsoccer.org.
STEP 4: A FINAL, MSYSA affiliated, team league roster
that is signed, stamped, and dated by your league registrar
must be physically in the MSYSA State Office, which is
located at 9401 General Drive, Suite 120, Plymouth, MI
48170, on or before 4:30 PM on August 14, 2009
Postmarks will not be accepted. Roster
additions may not take place following this deadline.
Failure to submit a roster by the established deadline
shall result in fines.
When preparing for State Cup roster deadlines, it is the team's responsibility to communicate with their League about obtaining a roster by the deadline. In the case of Michigan State Premier Soccer Programs teams, this may include requesting early pass cards. |

TERMS
& AGREEMENTS |
The
Head Coach or Team Manager must agree to the following
statement:

I
am entering the team named in this application
with the full understanding that all games
in this competition will be governed by the
following rules and in the following priority;
(i) the MSYSA Directives, Rules and Regulations
of the Competition as modified form time to
time by US Youth Soccer MSYSA State Cup Chairperson
or State Cup Chair Designee, with or without
notice, (ii) the Rules, Regulations and the
constitution of the MSYSA, (iii) the rules
as determined by US Youth Soccer, and (iv)
the FIFA Laws of the Game. I, further understand,
that the application fee is non-refundable
and rules/provisions of the US Youth Soccer
MSYSA State Cup tournament are subject to
change with or without notice.
I also understand that any and all US Youth Soccer
MSYSA State Cup materials (i.e. schedules, maps,
etc.) will not be sent via U.S. Mail. All
pertinent US Youth Soccer MSYSA State Cup material
and information will be e-mailed to each coach
or manager and all US Youth Soccer MSYSA State
Cup materials and information will be posted on
the MSYSA web site (www.michiganyouthsoccer.org). |
